In 2012, Dr. Thomas Cornwell founded HCCI with the revolutionary idea that high-quality home-based primary could transform our nation’s health care system. With almost twenty years as an exceptional house call provider and a passionate commitment to end-of-life care and chronic disease management, Dr. Cornwell soon garnered attention from philanthropists who shared his groundbreaking vision.
Thanks to the generous support of those donors, HCCI is now at the forefront of the national dialogue on providing primary care delivered to patients in their homes. We are proud to be advancing the practice of home-based medical care on a local, regional, and national scale. Our successful partnerships across the care continuum are helping us achieve the promise of health care’s Triple Aim.
