HCCI Regional Projects Overview

To address the need for home-based primary care (HBPC) by over 7 million people in the U.S., the Home Centered Care Institute (HCCI) brings a national and regional focus to expanding the HBPC workforce and increasing the awareness of this important type of care.

The goal of the regional projects is to identify, equip, and motivate a group of HBPC practices and providers in these regions —and those looking to expand or add HBPC as a new service — to participate in a 3-year program aimed at increasing the number of their regional residents receiving high-quality HBPC. In establishing and recognizing the practices’ Home Centered Care Champions, HCCI is looking to drive even greater impact with a more concentrated effort.

In Illinois, these practices also become part of the HCCI Practice Excellence Partner™ (PEP) Network. HCCI PEP sites are programs recognized nationwide by HCCI as leading house call programs and premier HBPC providers. HCCI looks to leverage this network as a framework for conducting quality improvement initiatives and supporting member practices through a collaborative learning community.
