Course Overview

In healthcare, teamwork is vitally important to the delivery of high-quality patient care. Ineffective communication and collaboration are responsible for up to 70% of the adverse events reported in healthcare. The home-based primary care (HBPC) team faces unique challenges that make effective teaming more difficult than in other healthcare providing teams, potentially leading to medical errors and adverse events. In this course, you will learn best practices to build an environment that promotes team cohesiveness and open communication to better support your patients and their families.
