The Home Centered Institute (HCCI), with support from the ELEA Institute, has launched the Innovation Award, an initiative developed to empower Illinois-based organizations that provide palliative care. This award is designed to help organizations expand their service offerings, enhance patient outcomes, and build long-term sustainability by implementing Chronic Care Management (CCM) services.
As part of this unique opportunity, award recipients receive a fully subsidized CCM Implementation Package. This includes access to expert-led webinars, personalized consultation sessions, targeted online courses and practical training resources all designed to support the successful integration of reimbursable CCM services.
The Innovation Award empowers participants to become leaders in home-based care while delivering greater value to the patients and families they serve.
Why Chronic Care Management in Palliative Care?
Chronic Care Management (CCM) helps organizations provide coordinated care to patients with serious or complex conditions. It improves symptom and medication management, reduces unnecessary hospital visits, and strengthens communication between patients, caregivers, and care teams. CCM also provides a sustainable, reimbursable revenue stream, allowing organizations to grow their services. Goals and Objectives
- Equip palliative care programs & providers with a subsidized CCM implementation package
- Designate and promote participants as Innovation Award recipients
- Train practices to use the HCCI Chronic Care Management Calculator
- Explore the implementation of other services such as Transitional Care Management, Telehealth, Caregiver Training, Social Determinants of Health, and/or Behavioral Health Integration
