AdventHealth House Call Providers

AdventHealth House Call Providers formerly AMITA Health Housecall Physicians was started by Dr. John Liu M.D. over 15 years ago when he recognized an increasing number of patients needing care in their home. The practice very quickly grew as he started to care for his colleagues patients in the medical group and soon found several local hospital systems referring to him as well. Dr. Thomas Mallidi M.D. joined Dr. Liu about 7 years ago and the practice has grown by leaps and bounds in both numbers of patients we serve and the areas we serve. Today our team consists of 3 physicians and 3 nurse practioners and a dedicated back office staff. We serve most of the northwest, west, and southwest suburbs as well as the northwest and southwest portions of Chicago. We are very closely integrated with Advent Palliative care, Advent Hospice, and Advent Home Health to offer a true team approach. We are quickly expanding our service lines and will soon be offering wound care specialty services in the home.

My name is Dr. John Liu.  I am the founder and lead physician for AdventHealth House Call Physicians.  We have been seeing patients in the Chicagoland area since 2005.  I started in 2005 as an office-based family physician in a practice that was over 80 years old.  At that time I began noticing that many of my elderly patients were unable to get to the office due to wide variety of debilities.  I decided to begin seeing some of my patients that could not come to the office on Wednesday afternoons- my day off.  Over the years, many of colleagues heard whispers that I was seeing patients at home and referrals from them began.  Over the last 18 years we have expanded to now having 3 physicians and 3 NPs seeing patients 5-6 days per week.

She also served as student ambassador at Georgetown during her term. After a few years in private practice, she found herself looking to do something more impactful in the healthcare field. She enjoyed seeing patients in senior facilities, and knew that extending to the home setting was the next step. She currently works as a home-based primary care nurse practitioner with the AdventHealth House Calls Physicians Group.

Dr. Thomas Mallidi has been practicing comprehensive primary care for almost a decade which includes both traditional office based practice as well as home-based primary care. He graduated from Rosalind Franklin University/The Chicago Medical School and attended residency at the family practice program at Adventist Hinsdale Hospital before entering practice in Hinsdale. He has been recognized as a leader amongst his peers in the medical group and patients. He currently is the Vice-Chair of primary care of AdventHealth Medical Group and has led many initiatives including the optimization of technology in practice. He has found home-based primary care to be the most rewarding of all of the facets of his practice and is excited about the bright future ahead for this field.


Comprehensive Care Program at the University of Chicago

Today, many patients have a primary care doctor who cares for them in the outpatient clinic. However, if they have to stay in the hospital, they usually get care from a doctor they do not know. This discontinuity in care can worsen patients’ health outcomes and experiences with care, particularly for those with multiple health problems. The Comprehensive Care Program includes two innovative care models that tackle this problem: the Comprehensive Care Physician (CCP) Program and the Comprehensive Care, Community, and Culture Program (C4P). Each offers patients with complex medical and social needs a comprehensive care physician who cares for them in both the hospital and the clinic. We firmly believe that strong relationships between patients and their doctors are key to better health and health care.

Meltzer’s research explores problems in health economics and public policy with a focus on the theoretical foundations of medical cost-effectiveness analysis and the cost and quality of hospital care. Meltzer has performed randomized trials comparing the use of doctors who specialize in inpatient care (“hospitalists”). He is currently leading a Centers for Medicaid and Medicare Innovation Challenge award to study the effects of improved continuity in the doctor patient relationship between the inpatient and outpatient setting on the costs and outcomes of care for frequently hospitalized Medicare patients. He led the formation of the Chicago Learning Effectiveness Advancement Research Network (Chicago LEARN) that helped pioneer collaboration of Chicago-Area academic medical centers in hospital-based comparative effectiveness research and the recent support of the Chicago Area Patient Centered Outcomes Research Network (CAPriCORN) by the Patient Centered Outcomes Research Institute (PCORI). Meltzer received his MD and PhD in economics from the University of Chicago and completed his residency in internal medicine at Brigham and Women’s Hospital in Boston. Grace Berry LaShore, MD, is an Internal Medicine physician in the Comprehensive Care Program in the Section of Hospital Medicine, specializing in the care of complex adult patients in both the inpatient and outpatient settings. After earning her undergraduate degree in Biology at Yale University, she received her medical degree at Columbia University College of Physicians and Surgeons, and completed her residency at NY Presbyterian Hospital- Columbia University. Through geriatric house call dentistry, clinicians provide dental care when, where and how it meets the individual patient’s needs. Joy V. Poskozim DDS FSCD CDP, a general dentist and national speaker teaches dental and medical audiences what they need to know and what they need to do to provide mobile dental care to patients in the comfort of their home environments. Dr. Joy’s true passion is being an advocate for the geriatric population. She is the dental director for several nursing care facilities as well as a house call dentist. She has been in private practice for over 20 years, practicing integrative health general dentistry at her office on the northwest side of Chicago, and has been performing dental procedures on the home-bound for 10 years. Through this experience Dr. Poskozim has earned her Fellowship with the Special Care Dental Association Geriatric Council. Dr. Joy has been treating senior community residents in the comfort of their own rooms for over ten years. During this time, not only has she been making house calls across Chicagoland, but also educating certified nursing assistants, caregivers, nurses, and administrators. She is passionate about enhancing patient quality of life and embodies the values of communication, education, safety and trust. She started her career in nursing in 1999 when she graduated from Olive Harvey College with her ADN, years later she completed her BSN from Kaplan University in 2006 followed by her MSN/FNP in 2015 from Governors State University in Illinois. Among her many accomplishment she was published in the Worldwide Leaders in Healthcare in 2021. She believes in treating the whole person, mentally, physically and spiritually.
