Practice Assessments

Chart Audits – Coding and Documentation

A nationally certified medical auditor will conduct a formal audit of the practice’s patient charts, providing confidential, qualified feedback on your providers’ documentation practices and coding.

  • Ensure that your practice’s coding accurately represents each patient’s diagnosis and the services provided.
  • Increase revenue by appropriately using available codes, with proper documentation, to be reimbursed in a timely manner for the services provided to patients and their families.

Onsite Coaching – Coding, Billing, and Documentation

A nationally certified medical auditor will assess your practice’s coding and billing procedures, review a sampling of patient charts, and evaluate the quality of provider documentation to identify gaps and targeted opportunities for improvement. Our expert(s) will then travel to your practice to provide onsite coaching and to recommend, develop, and/or implement semi-custom documentation tools and training support plans aimed at enhancing documentation and increasing reimbursement.

Dr. Thomas Cornwell, Chief Executive Officer

Thomas Cornwell, MDDr. Cornwell founded the Home Centered Care Institute to create a national education and research organization whose mission is to expand house call programs and the workforce nationally. His efforts have received extensive local and national media attention including features in the New York Times, TIME Magazine and PBS. In 2017, Fast Company named him a top 100 Most Creative People in Business for bringing health care into the home.

Dr. Cornwell founded his practice, HomeCare Physicians, in 1997, and he has personally made over 33,000 house calls. His innovative care has received numerous awards including the first ever House Call Doctor of the Year, and the Pride in the Profession award given by the American Medical Association to four heroes in health care in America bringing pride to the medical profession. The American Geriatric Society presented Dr. Cornwell with the 2015 Arnold P. Gold Humanism in Medicine award. He is a past-president of the American Academy of Home Care Medicine. Dr. Cornwell has mentored hundreds of medical students, residents, nurse practitioners, and physician assistants to enhance their confidence and competence in home-based primary care.

 

Dr. Paul Chiang, Senior Medical and Practice Advisor

Paul Chiang, MDDr. Chiang focuses his time on developing curriculum, teaching current and future providers and presenting locally and nationally on the value of home-based primary care.

Dr. Chiang is also Medical Director for HomeCare Physicians, and he has made over 32,000 house calls to more than 2,800 patients over his 18+ year career. Dr. Chiang attended medical school at the University of Illinois in Chicago and completed his residency program at the Medical College of Wisconsin in Milwaukee. He holds a Master of Arts in Bioethics. His practice and work have been featured on ABC News Chicago and in northeast Illinois’ Daily Herald. In 2014, Central DuPage Hospital named him as one of 50 Unsung Heroes.

 

Dana Crosby, Director, Research & Practice Improvement

Dana CrosbyDana brings a 20+ year portfolio of success driving business, human resources and consulting strategies, functions, and initiatives for organizations across diverse industries. Prior to joining HCCI, Dana led a global Human Capital Management consultation and implementation services practice with a portfolio inclusive of Fortune 500 companies. Dana holds a Bachelor of Arts degree in both Business Administration and Psychology. Along with her expertise in consulting and human resources, she brings a personal passion and professional commitment to the mission of HCCI.

 

Brianna Plencner, CPC, CPMA, Manager, Practice Improvement

Brianna Plencner, CPC, CPMABrianna has deep knowledge and experience in house calls practice management having focused her career in Primary Care and Home Care Medicine. Brianna is a certified coder since 2014, Certified Professional Medical Auditor as of 2018, and holds a diploma in Medical Insurance Billing and Coding. Prior to joining HCCI, Brianna served as the Practice Manager for HomeCare Physicians, a home-based primary care practice which is part of Northwestern Medicine in the Chicago, IL area; she also has experience working with inpatient hospital and critical care physician practices. In addition to her understanding of practice operations, Brianna excels at developing workflows and efficiencies for home-based primary care practices. Brianna has strong communication skills with experience bringing front and back office teams together.
