Course Overview

This course equips home-based primary care (HBPC) providers and administrators with the essential knowledge and skills to implement and sustain the Advanced Primary Care Management (APCM) model. Participants will explore how APCM uniquely supports HBPC delivery, learn to operationalize best practices, and effectively navigate billing and documentation requirements. The course also provides strategies to measure program success through clinical, quality, and financial outcomes.

Audience

This online course is intended for home-based primary care providers, as well as coders, practice managers, and operations staff.

Estimated Time to Complete

30 minutes. In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.
