Julie Sacks is the president and chief operating officer (COO) of the Home Centered Care Institute (HCCI), a national nonprofit organization focused on scaling home-based primary care and bringing it into the healthcare mainstream. In the role, Sacks is responsible for evaluating strategic opportunities and partnerships, developing relationships with donors and foundations, and serving as spokesperson for the organization, in addition to serving as the primary liaison to the HCCI board of directors.
Since joining HCCI in 2015 as vice president of operations and advancement, Sacks has orchestrated the development and implementation of the organization’s annual plan and strategic roadmap, while managing HCCI’s executive team and the organization’s daily operations.
Prior to joining HCCI, Sacks was senior director of programs and services and director of the National Young Onset Center for the American Parkinson Disease Association. She graduated with a bachelor’s degree in advertising from the University of Texas at Austin, and then earned a master’s degree in social work from the University of Texas at Arlington.
During Sacks’ tenure at HCCI, the organization has made significant strides toward creating universal access to house call programs and advocating on behalf of home-based primary care (HBPC). With Sacks’ support, the organization participated in a campaign that resulted in the continuation of the Centers for Medicare & Medicaid Services Independence at Home Medicare Demonstration and development of new payment models for home-based care.
With over 25 years of experience in continuing medical education (CME) and health care quality improvement, Singleton has held senior staff positions at medical specialty societies and has also served as an executive consultant to health care organizations at national and state levels. She has a graduate degree in adult professional education/human resource development and brings extensive knowledge and expertise in educational design, research, quality measurement, and health care outreach.
In her role as Chief Programs Officer at the Home Centered Care Institute, Singleton oversees the development, implementation, and evaluation of all educational programming, research initiatives, and a wide range of projects, services, and outreach aimed at expanding access to high-quality home-based primary care (HBPC) for patients who need it. She is passionate about leveraging strategic partnerships and working with like-minded collaborators to grow the HBPC workforce and improve the sustainability of house call programs that serve frail, older adults and patients of any age who are medically complex, require advanced care for serious illness, and/or have injuries or conditions causing them to be homebound or home limited.
Dr. Paul Chiang focuses his time on developing curriculum, teaching current and future providers and presenting locally and nationally on the value of home-based primary care.
In addition to his role at HCCI, Dr. Chiang serves as Medical Director for Northwestern Medicine HomeCare Physicians (HCP), a suburban Chicago practice focused on delivering care to medically complex patients in their homes. HCP has made more than 131,000 house calls to home-limited patients since its founding in 1997, and Dr. Chiang has personally made over 38,000 house calls to more than 3,800 patients during his 23-year career. His practice and work have been featured on ABC News Chicago and in northeast Illinois’ Daily Herald. In 2014, Central DuPage Hospital named him as one of 50 Unsung Heroes. In October 2019, Dr. Chiang received the “House Call Doctor of the Year” award from the American Academy of Home Care Medicine.
Leading a dedicated team, Crosby oversees initiatives aimed at enhancing the delivery of home-based primary care. In her role, Crosby manages relationships with learners, faculty, consulting clients, and other stakeholders, fostering communication and collaboration to drive meaningful change. She brings over 20 years of experience in business, human resources, and consulting, with a proven track record of success across diverse industries. Her expertise in driving business and consulting strategies has been invaluable in her current role, where she leads practice development and engagement activities, including education, consulting services, pilot projects, and the HCCIntelligence™ program.
Before joining HCCI, Crosby led a global Human Capital Management consulting practice, working with Fortune 500 companies to implement strategic initiatives. Crosby holds a Bachelor of Arts degree in Business Administration and Psychology, as well as a certification in Diversity, Equity, and Inclusion in the Workplace. Her academic background, coupled with her professional experience, equips her to effectively lead HCCI’s efforts in advancing home-based care.
Warda oversees marketing, communications and public relations for the Home Centered Care Institute. In this role, he has developed a strong brand presence for the organization and secured a variety of prominent media placements, including ABC News, CNN, Crain’s, Forbes, Health Affairs, Home Healthcare News and Hospice News, and NPR. He also manages HCCI’s portfolio of marketing channels, including publications, social media, and videos.
Warda brings over 25 years of experience in communications, marketing, and public relations across multiple industries, including aerospace, defense, energy management and automation, finance, healthcare, insurance, manufacturing, pharma, and retail. Prior to joining HCCI in 2019, he served in a variety of leadership roles at Fortune 100 companies, including Allstate, Baxter, Boeing, Caremark, McDonald’s, Schneider Electric and Walgreens Boots Alliance.
Warda holds a Bachelor of Arts degree in English from Loyola University Chicago where he served on their Dean’s Communication Advisory Board and was an adjunct professor. He has also been a guest lecturer for DePaul University of Chicago.
