At the annual Scientific Meeting of the American Geriatrics Society, May 4-6 in Long Beach, California, Dr. Paul Chiang, Senior Medical and Practice Advisor, HCCI, and Medical Director, Northwestern Medicine HomeCare Physicians, was among the presenters of the Choose Your Own Adventure: Navigating the Home-Based Care Continuum in Clinical Practice workshop.

More than 80 people attended the session, sponsored by the organization’s Home-Based Primary Care Special Interest Group and Society for General Internal Medicine Special Interest Group. The workshop followed a patient through multiple home-based care settings and allowed participants to make medical decisions in each environment together with experts in the field.

“It was energizing to see so many professionals in the geriatrics community come together to learn more about the home-based care continuum and why it is an essential component of systems-based practice,” said Melissa Singleton, Chief Learning Officer, HCCI. “Faculty took learners through the full range of home care settings and models, including home-based primary care, home-based palliative care, assisted living, hospital at home, and sub-acute rehab to home.”
