HCCI Employee Orientation: Week 1

This week is all about helping you get settled and confident in your role. You’ll connect with your supervisor, get familiar with our systems, and begin exploring the core concepts that guide home-based primary care.

Learning Objectives

Upon completion, you will be able to:
- Describe the fundamentals of home-based primary care, its value to payers, patients, and caregivers, and the role of house calls in patient-centered care.
- Discuss the various components involved in patient assessments in a home setting, recognizing key factors that influence care delivery.
- Explain the importance of diversity, equity, and inclusion in home-based care and how to apply these principles in practice.

Audience

These online courses and educational resources are intended for physicians, advanced practice providers (NPs, PAs), nurses (RNs, LPNs), practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.
