HCCI Employee Orientation: Week 2

This week is focused on meeting with key team members to gain deeper insight into HCCI’s work. You will also begin diving into what it means to manage the needs of medically complex, homebound patients and explore specialized topics like medication management, palliative care, and interdisciplinary teams.

Learning Objectives

  • Upon completion, you will be able to:
    • Examine a management model for treating homebound patients with multiple chronic diseases.
    • Identify the challenges and opportunities experienced by HBPC providers in the complicated medication management for their patients.
    • Discuss the principles of home-based palliative care and recognize the importance of advance care planning for patients with serious illness.
    • Describe the intersection of palliative care and home-based primary care and how to support patients with complex needs.
    • Recognize the components of a successful interdisciplinary HBPC team and how to contribute effectively within one.

Audience

These online courses and educational resources are intended for physicians, advanced practice providers (NPs, PAs), nurses (RNs, LPNs), practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.
