This unique two-day field experience is designed to allow providers and practice managers to shadow trained preceptors and acquire new knowledge and skills in house call operations and clinical care for medically complex patients in the home. The HCCI House Call Practicum™ offers an individualized learning opportunity at our Practice Excellence Partner (PEP)™, which are programs recognized nationwide as premier providers of home-based primary care.
On Day 1, you will accompany your PEP Preceptor on house calls and observe the various steps followed by the practice prior to, during, and after each home visit. Your preceptor will also describe or demonstrate the practice’s approach to the management of urgent issues in the field, after-hours coverage, communication with practice staff and care coordination.
On Day 2, you will observe front and back office operations, including scheduling, new patient intake, and appointment confirmation; preparing and maintaining supplies and packing the medical bag; triage and communication; billing, coding, and revenue cycle review; analytics; patient selection criteria and service area; marketing and outreach activities; staffing and productivity.
Dates: This session will be held September 9 - 10, 2024.
Location: Rush@Home, on the campus of Rush University Medical Center, Chicago, IL.
Upon completion of the HCCI House Call Practicum, you will be able to:
1. Describe the day-to-day practices of one of the nation's premier home-based primary care programs.
2. Demonstrate enhanced knowledge and skills in HBPC practice operations and efficiency.
3. Apply effective strategies for providing optimal, patient-centered care for medically complex patients in the home.
This course is intended for physicians, advanced practice providers, nurses, practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.
16 hours, on two consecutive days of field experience.

Serving the West Side of Chicago, IL, and selected western suburbs, Rush@Home is a growing house calls program that offers longitudinal home-based primary care to patients with 2+_chronic conditions, functional impairment, and past emergency department and hospital utilization. When this site is selected, the HCCI House Call Practicum™ gives you access to the program’s clinical and practice operations, as well as the interdisciplinary team comprised of primary care providers, certified medical assistants, a lead medical assistant, a nurse, a social worker, and the program manager.

Cancellation & Refunds
Although unlikely, HCCI reserves the right to cancel or reschedule a House Call Practicum when necessitated by certain circumstances. If HCCI cancels a House Call Practicum, you will have the option to either a) transfer tuition paid to another HCCI activity or b) receive a full refund. You must inform HCCI of your choice of either the transfer or refund within two (2) weeks of the cancellation. If no such notification is received, HCCI will issue a refund.
If you wish to cancel your registration, you must send notice of cancellation via e-mail to education@hccinstitute.org. If notice is received prior to 5:00 pm CST two (2) weeks prior to the first day of the House Call Practicum, you will be refunded the amount of the tuition paid, less a $150 administrative fee. In lieu of a refund, the full amount of the tuition paid may be applied to a future HCCI activity. The request to apply tuition credit to a future HCCI activity must be made in writing by e-mail within two (2) weeks of the cancellation, or a refund will be issued less the $150 administrative fee.
Cancellation requests submitted after the deadline noted above, will not receive a refund. No-shows will not be eligible for a refund or transfer of tuition.
Other
Requests to transfer your registration to another person will be considered on a case-by-case basis. Transfer requests must be made in writing no later than 5:00 pm CST one (1) week prior to the first day of the House Call Practicum. The House Call Practicum is designed as a comprehensive two-day activity, and enrollment may not be “split” between participants. Likewise, there is no single-day tuition.
HCCI is not liable or responsible for any travel, lodging or work expenses incurred in the event of cancellation that is initiated by either HCCI or the registrant.
