Course Overview

This course offers an in-depth exploration of Chronic Care Management (CCM) service, specifically designed for providers and non-clinical professionals delivering care in the home-based primary care setting. Building on foundational CCM knowledge, participants will examine strategies to enhance care coordination, maximize billing efficiency, and integrate CCM into broader practice workflows.

This online course is intended for home-based primary care providers, as well as coders, practice managers, and operations staff.

30 minutes. In order to earn credit for participation in this activity, the learner must complete a 5-question post-test with a passing score of 80% or better with unlimited attempts.
