HCCI Careers
Be The Difference
The Home Centered Care Institute (HCCI) is a mission-driven nonprofit dedicated to increasing patient access to home-based primary care (HBPC), particularly for medically complex patients who are best cared for at home. HCCI is passionately committed to its vision of bringing HBPC into the healthcare mainstream, and we are proud to be advancing the field of HBPC through education, consulting, research, and advocacy.
At HCCI, though, we are not simply asking you to make a difference; we are asking that you be the difference. By joining HCCI, you can be part of a mission-guided team ensuring that vulnerable individuals receive essential healthcare services at home. Because, as we see it, the future of healthcare is in the home.
To support us in achieving this mission, HCCI is looking to fill several key roles below. If interested in applying, please click on the link below or, if you know of someone who might be interested and a good fit, please share the link with them.

Director, Market Growth & Strategic Partnerships
The Director, Market Growth & Strategic Partnerships will architect the expansion of a movement. This person will identify and nurture long-term alliances with health systems, ACOs, payers, and private practices, providing them with the intellectual capital (education) and business intelligence (data analytics) they need to succeed in the future of healthcare. This is a leadership position for someone who enjoys building something durable - relationships and revenue - while advancing a mission that matters. The position carries clear revenue accountability and ambitious growth goals, while requiring a consultative, mission-aligned approach that reflects HCCI’s nonprofit values and standing in the home-based care community. This position does not manage a team, but rather, drives and owns the process themself, in collaboration with other HCCI team members.
HCCI seeks a growth-oriented professional who understands that lasting revenue is built through trust, credibility, and long-term partnerships and not just transactional selling alone. The Director will develop and steward relationships with provider organizations, population health leaders, and healthcare partners, translating HCCI’s education, consulting, and data analytics offerings into sustained value for the field.
Director, Education
The Director, Education directs the planning, design, implementation, and evaluation of continuing medical education activities (e.g., in-person workshops, live webinars, Internet enduring materials) and resources that advance the knowledge and skills of healthcare professionals and organizations delivering home-based medical care. Analyzes needs assessment data, develops learning objectives, and selects instructional methodologies aimed at achieving desired outcomes in continuing medical education (CME)/continuing education (CE) activities. Confirms participation of suitable Faculty/Subject Matter Experts (SMEs) and facilitates their development of education activities and resources containing accurate, timely, and relevant content. Ensures compliance of HCCI’s CME/CE activities with the accreditation criteria of the Accreditation Council for Continuing Medical Education and with the American Medical Association’s requirements for AMA PRA Category 1 Credit™.
