Home Centered Care Institute Names Founder Dr. Thomas Cornwell Chief Medical Officer

Schaumburg, IL — Home Centered Care Institute (HCCI) today announced that Dr. Thomas Cornwell, the organization's founder and original CEO, has been named Chief Medical Officer. Dr. Cornwell stepped down as CEO in 2020 but has remained closely connected to HCCI, serving as Executive Chairman of the Board.

For more than three decades, Dr. Cornwell has dedicated his career to ensuring that frail, medically complex, homebound patients receive the care they deserve in the place they call home. Over that time, he has personally completed more than 35,000 house calls, becoming one of the nation's leading advocates for home-based primary care. As Chief Medical Officer, he will help advance HCCI's mission through education, clinical leadership, advocacy, and the training of the next generation of home-based primary care clinicians.

"I've spent my career in patients' living rooms and kitchens, and I've seen firsthand what happens when healthcare comes to people who can no longer come to healthcare," said Dr. Cornwell. "For years, the case for this model was primarily a moral one—it was simply the right way to care for our sickest and most vulnerable patients. Today, value-based payment models are finally recognizing what we've known all along: better care in the home leads to better outcomes, a better patient and caregiver experience, and lower costs. That creates an unprecedented opportunity to bring this model to many more patients across the country."

Earlier this year, Dr. Cornwell joined Advanced Care Innovations (ACI) as Chief Medical Officer, where he works with home-based primary care organizations participating in value-based care models. His work at ACI focuses on helping practices achieve clinical excellence and financial sustainability while serving some of the nation's most medically complex patients.
