The House Call Advisor Network (HCAN) is a dynamic community of clinicians, operational leaders, and innovators who believe in the power of house calls to transform lives. HCAN exists to strengthen house calls nationwide by connecting experienced professionals with organizations seeking guidance, education, and innovation.
Why HCAN Exists
Over 7 million Americans are homebound or home-limited, yet most lack access to high-quality primary care. Home-based primary care reduces hospitalizations, lowers costs, and improves patient outcomes, but fewer than 15% of those in need receive it. HCAN was created to help address this gap by leveraging the knowledge and experience of seasoned experts who have firsthand experience in home-based medical care. Through mentorship, advisory support, and collaboration, HCAN helps health systems, independent practices, and payers implement and expand home-based medical care programs.
