Course Overview

High-risk or chronically ill patients often require care from multiple individuals, including home-based primary care (HBPC) teams. Providing safe, high-quality care that puts the patients’ needs first requires thoughtful and strategic care management and coordination among HBPC team members.

In this course, you will learn how to support and develop best practices for meeting the individual needs of patients through care management and coordination.

  • Identify gaps in care, and target care management strategies to proactively manage complex populations.
  • Describe interdisciplinary team member roles in home-based medicine and strategies to engage in top-of-licensure work.

This online course is intended for members of the clinical team caring for homebound patients, including nurses and social workers.
