Home-based primary care (HBPC) brings the expertise of physicians, nurse practitioners and physician assistants and the advanced technology of a health care clinic directly to chronically ill, medically complex, functionally homebound patients, providing comprehensive care in their home.

Home-Based Primary Care Meets Healthcare’s Triple Aim

  1. Better patient outcomes and improved quality of life
  2. Higher patient and caregiver satisfaction
  3. Provides (better) care at a lower cost

Your Donation to HCCI supports these mission driven initiatives:

TRAINING

Your support helps HCCI develop and teach a curriculum of instructor-led training workshops at our seven Centers of Excellence partners around the country. HCCI also develops online courses in the field of home-based primary care.

CONSULTING

Your donations support HCCI’s consulting efforts – educating and training individuals and organizations interested in starting or growing a home-based primary care practice.

RESEARCH

To solidify its place as a unique model of care, it is critical that home-based primary care (HBPC) be built on evidence-based practices. HCCI is working to develop best clinical and operational practices through a new data registry. Our Centers of Excellence (COE) partners as well as house call programs that participate in HCCI training will be invited to contribute to the data registry and best practice development. Research and data are the key to payment model reform.

ADVOCACY

In order to draw more clinicians to the field, together with the American Academy of Home Care Medicine, HCCI is supporting payment model reform efforts to see that physicians and APP’s who serve patients in their homes are compensated similarly to practitioners who see patients in their office and hospital-based practices.
