HCCI holds another successful meeting of the Chicago Home Centered Care Network and Learning Collaborative on April 14, 2016 at Northwestern University.  Representatives from health systems and academic institutions from across the Midwest attended the session in which national leaders in Home Centered Care presented on the shift to value-based care and the benefit of house call programs for patients and payors.
