HCCI Employee Onboarding: Week 5

This week focuses on the clinical and social complexities of caring for patients in the home. You'll learn how we train providers to navigate dementia behaviors and other mental health conditions for homebound patients. You'll also explore available resources for managing durable medical equipment (DME).

Learning Objectives

  • Upon completion, you will be able to:
    • Recognize and discuss mental health conditions and neuropsychiatric symptoms in homebound patients.
    • Discuss care management for patients with complex medical and psychosocial needs.
    • Describe elements of social determinant screening, the use of durable medical equipment resources, and the importance of accurate provider documentation.

Audience

These online courses and educational resources are intended for physicians, advanced practice providers (NPs, PAs), nurses (RNs, LPNs), practice managers/administrators, social workers, residents/fellows, medical assistants, and other practice operations staff.
