Welcome to the webinar series, “Demystifying Value-Based Care for House Calls." This 12-week, virtual, and self-paced series can be accessed through the HCCI Learning Hub™. Below is the recommended course schedule along with the estimated time of completion for each course and CME credits available to you. Please note, CME credits are granted after completing all three modules and evaluation survey. After the completion of each session, please join HCCI subject matter experts for a 90-minute webinar to discuss any questions you may have regarding the course material. Learning Objectives

Upon completion of this activity, learners will be able to:

  • Discuss value-based payment opportunities currently available to HBPC programs, including strategies for patient identification, quality measurement, contract negotiation, and establishing key partnerships.
  • Apply recommended practices for staffing and productivity, efficiency, telehealth, risk stratification, and coordination with other providers.
  • Provide high-quality, cost-effective patient care and successfully manage conditions of specific concerns in value-based payment arrangements, including dementia, congestive heart failure, and polypharmacy.
  • Manage transitions of care for homebound patients within the domains of accountability, communication, assessing needs and goals, medication management, and safety.
