The Illinois House Call Project Practice Assessment Kick-off meeting reviewed the purpose, objectives, benefits, value, and key milestones of the Practice Implementation Model and the Practice Assessment phase. This meeting was required for Champions to attend. Meeting resources include the presentation slide deck and the Data Request Form, which is to be completed and submitted by each practice no later than July 1st, 2025.
