Overview, Requirements, and Benefits

In response to the demand and urgent need to grow and support the home-based primary care (HBPC) workforce in Illinois, the Home Centered Care Institute (HCCI) is implementing the Illinois House Call Project.  The goal of the project is to identify, equip, and motivate a cohort of practices dispersed across Illinois currently providing HBPC —and those looking to expand or add HBPC as a new service — to participate in a 3-year program aimed at increasing the number of Illinois residents receiving high-quality HBPC.

These practices will become part of the HCCI Practice Excellence Partner (PEP) Network. HCCI PEP sites are leading house call programs recognized nationwide as premier HBPC providers.

Through this project, HCCI will:

  • Identify up to 20 Illinois practices as members of the HCCI Practice Excellence Partner™ Network and equip them with customized assessment, training, market intelligence, and resources to collectively enroll and serve a total of 3,000 new HBPC patients by 2024, while — at a minimum — maintaining their current active patient census.
  • Elevate the awareness and promote the value of HBPC for the healthcare community, the public, payers, policymakers, and other key stakeholders, in Illinois.
  • Leverage the HCCI Practice Excellence Partner™ Network to establish a framework for conducting quality improvement initiatives and supporting member practices through a collaborative learning community.

HCCI Practice Excellence Partner™ Network Practices will:

  • Commit to a three (3) year term through 12/31/2024; estimated time required is 10-12 days per year
  • Designate up to three (3) Champions to participate in and complete the following complimentary training:
    • Essential Elements of Home-Based Primary Care™– two-day virtual workshop, June 2-3, 2022
    • Advanced Applications of Home-Based Primary Care™– two-day virtual workshop, July 21-22, 2022
    • Selected online courses from the HCCI Learning Hub™
    • Two (2) one-day, in-person training sessions (one in Year 1, one in Year 2) customized for Home Centered Care; Content to include:
      • Leadership, Team Dynamics, and Change Management
      • Supply and Demand of HBPC in Illinois
      • Preparing for Value-based Payment
      • Mastering the Art of Preceptorship
      • Media and Public Relations
      • Marketing
      • Community Resources
      • Using HCCI Curriculum and Resources
      • Quality Improvement
    • Participate in two (2) comprehensive Practice Assessments (at the start, and again at the end of the project), including execution of a Business Associate Agreement (BAA) to support adherence to HIPAA compliance
    • Commit to hosting and precepting individual learners (up to 4/year) for the HCCI House Call Practicum™, a unique two-day field experience that allows providers, practice managers, and others to shadow your team on house calls and in observing front and back office operations
    • Adhere to deadlines for completion of milestones and reporting of performance metrics, which will be shared with and evaluated in partnership with HCCI



As a designated Champion and HCCI Practice Excellence Partner™ Network Practice, you will:

  • Serve a key role in Illinois and in the national movement to improve patients’ access to high-quality home-based primary care. You will make a difference!
  • Earn up to 26.5 AMA PRA Category 1 Credits™ at no charge through participation in HCCI’s four-day Virtual Boot Camp in Home-Based Primary Care (HBPC), which includes Essential Elements of Home-based Primary Care™ (June 2-3) and Advanced Applications of Home-based Primary Care™ (July 21-22)
  • Earn additional AMA PRA Category 1 Credits™ at no charge through participation in selected HCCI online courses and the one-day training sessions
  • Have access to HCCI’s Research Services to visualize key data in real-time, including:
    • Supply Assessment by Region
    • Market Assessment by Region
    • Service Area/Radius Map by Driving Time
    • Referral Sources and Patterns
    • Hospices, Home Health, and Assisted Living Facilities
  • Gain insight from two (2) comprehensive Practice Assessments (at the start, and again at the end of the project), including:
    • Chart audits
    • Customized market analysis
    • Individualized Practice Development Plan (PDP)
    • Practice Development Plan (PDP) Outcomes Report
  • Be granted a limited license to use HCCI curriculum, resources, and tools for teaching purposes
  • Be recognized on the HCCI website as one of the state’s leading house call programs and a premier provider of home-based care
  • Have an exclusive opportunity to apply for and receive capital funding from HCCI to support practice growth and sustainability goals identified during the initial Practice Assessment
  • Have exclusive opportunities to be connected to HCCI payer partner(s) for consideration of value-based contracts
