The practice of healthcare today is in a state of tremendous transition. Changing demographics, rising costs, and ever-changing policies and fee structures – not to mention the COVID-related strain on already stretched resources – have left healthcare providers struggling to provide cost-effective, quality care. To address this situation, Home-Based Primary Care (HBPC) offers a proven alternative model of care for the frail, complex, chronically ill and disabled aging population by meeting healthcare’s quadruple aim:

  • Improved patient outcomes
  • Improved patient and caregiver satisfaction
  • Reduced cost
  • Improved experience for physicians, NPs, PAs, and other practitioners

The Illinois House Call Project

In response to the demand and urgent need to grow and support Illinois’ HBPC workforce, the Home Centered Care Institute (HCCI) is implementing the Illinois House Call Project.  The goal of the project is to identify, equip, and motivate a cohort of practices dispersed across Illinois currently providing HBPC —and those looking to expand or add HBPC as a new service — to participate in a 3-year program aimed at increasing the number of Illinois residents receiving high-quality HBPC.

These practices will become part of the HCCI Practice Excellence Partner (PEP) Network. HCCI PEP sites are programs recognized nationwide by HCCI as leading house call programs and premier HBPC providers.

Through the project, HCCI will:

  • Identify and equip up to 20 Illinois practices as members of the HCCI Practice Excellence Partner™ Network with customized assessment, training, market intelligence, and resources to collectively enroll and serve a total of 3,000 new HBPC patients by 2024, while — at minimum — maintaining the current active patient census.
  • Elevate the awareness and promote the value of HBPC for the healthcare community, the public, payers, and policymakers, and other key stakeholders, in Illinois.
  • Leverage the HCCI Practice Excellence Partner™ Network to establish a framework for conducting quality improvement initiatives and supporting member practices through a collaborative learning community.

Participation in the Program

Each participating practice will designate up to three (3) provider(s) and/or other professional(s) to serve as Champions for the project and receive specialized training from HCCI to spread awareness of HBPC within their communities and lead local education of other providers, practice/operations staff, and the public.

Benefits of Participating

Practices selected to be part of the Illinois House Call Project will receive the following benefits:

  • Comprehensive Practice Assessments conducted at the start and end of the 3-year project, including:
    • Chart audits
    • Customized market analysis
    • Individualized Practice Development Plan (PDP)
    • Practice Development Plan (PDP) Outcomes Report
  • Opportunities to apply for and receive capital funding from HCCI, based on needs identified through the baseline Practice Assessment, to support the practice’s growth and sustainability goals
  • Complimentary training and services for up to three (3) Champions:
  • Access to HCCI’s Research Services to visualize key data in real time, including:
    • Supply Assessment by Region
    • Market Assessment by Region
    • Service Area/Radius Map by Driving Time
    • Referral Sources and Patterns
    • Hospices, Home Health, and Assisted Living Facilities
  • Opportunities to be connected to HCCI payer partner(s) for consideration of value-based contracts
  • Recognition as an HCCI Practice Excellence Partner™ including mention of the Champion(s) at each participating practice
