The Crossroads of Care: Managing Serious Illness in the Home series, offered from June 2023 to March 2024 and supported by the Elea Institute, helped hospice and palliative care programs discover the benefits of home-based primary care (HBPC), and learn how to successfully bring HBPC together with hospice and palliative care to provide a full-service solution for managing patients with serious illness, medical complexity, and/or chronic disease.
